Direct Deposit Form (SF1199A)

We are paid by two completely different systems and need to send BOTH organizations a copy of your Direct Deposit Form.

1. Send a copy of your Direct Deposit form to the team admin (without your social security number. she will add that and send it on as an encrypted email to the travel system.

2. You need to log onto myPay and submit your direct deposit form to them directly.
